Without scripting this is a form, not a lookup. The bands themselves are on the IRIS stages page: creatinine under 1.6 / 1.6 to 2.8 / 2.9 to 5.0 / above 5.0, SDMA under 18 / 18 to 25 / 26 to 38 / above 38, and if the two disagree, the higher one wins.

A stage is where she is now, not a clock. How long, by stage, with the studies. Save the sheet in her notebook if you want the next one sitting next to it.
